Ed Choice Scholarship

Registration of Incoming Kindergartners

If you live in the city of Dayton school district you may be eligible for an EdChoice Scholarship.

Submit to Immaculate Conception School office:

EdChoice Request Form education.ohio.gov (list of eligible schools will be on this website)
birth certificate
last four digits of the child’s social security number
immunization record
proof of residency (a utility bill in your name showing the address)
baptismal record (if applicable)
custody papers (if applicable)
The Income Verification Form must be completed online. education.ohio.gov

Registration of Grades 1-8

If your child is already in a Dayton Public School that is listed academic crisis you may qualify for an EdChoice Scholarship (all paperwork for EdChoice Scholarship must be sent in to the school office to be entered into the State of Ohio system). education.ohio.gov will have the forms and list of Dayton Public Schools eligible.

If you are not eligible for an EdChoice Scholarship you may qualify for an EdChoice Expansion Scholarship. The Income Verification must be completed online. education.ohio.gov
